Objective: to study the interaction of different types of personality disorders (somatoperceptive accentuation) with cardiovascular diseases (on the model of arterial hypertension and coronary heart disease), taking into account the course of heart disease and therapy adherence. Methods: patients with arterial hypertension and coronary heart disease from the department of cardiology underwent cardiological examination using standart techniques, clinical interview with psychiatrist. Criteria for assessing prognosis of cardiovascular disease were developed. Results: 3 specific psychosomatic\cardiopersonified syndromes, formed by interaction of personality disorders and somatic illness, were obtained: the first — with a favorable course of cardiovascular disease, congruent somatoperceptive accentuation (somatotony/neuropathy) of personality disorder; second — with adverse course of cardiovascular disease, provoked by somatopercepive personality disorder (body image aloof); third — with unfavorable course of cardiovascular disease, comorbid neuropathic accentuation of personality disorder. Conclusion: favorable course of cardiovascular disease in condition of congruent somatoperceptive accentuation: personality disorder helps to improve the prognosis of the disease (prevention of complications, high therapy adherence). Unfavorable course in condition of body image aloof\neuropathy: personality disorder is associated with worsening of the prognosis (complications, low therapy adherence).</p></abstract><trans-abstract xml:lang="ru"><p>В основу рабочей гипотезы положено представление о расстройстве личности и сердечно-сосудистых заболеваниях (с учетом течения и приверженности к терапии) как об изначально независимых рядоположенных дименсиях (в противоположность мнению о расстройстве личности как факторе предиспозиции к сердечно-сосудистым заболеваниям), взаимодействие которых формируется на всем пространстве кардиоваскулярной патологии. Цель исследования: изучение взаимодействия личностных аномалий (соматоперцептивные акцентуации) с сердечно-сосудистыми заболеваниями с учетом динамики кардиоваскулярной патологии. Материалы и методы: кардиологическое (с использованием стандартных методик и критериев оценки динамики сердечно-сосудистой патологии), неврологическое, психопатологическое обследование пациентов с ишемической болезнью сердца и артериальной гипертонией. Результаты: Выделено 3 кардиоперсонифицированных синдрома, формирующихся при взаимодействии соматической патологии и расстройства личности: сердечно-сосудистые заболевания с благоприятным, конгруэнтным соматоперцептивной акцентуации (соматотония/невропатия) расстройства личности течением; сердечно- сосудистые заболевания с неблагоприятным, провоцированным соматоперцептивным расстройством личности (соматодисгнозия с отчуждением образа тела) течением; сердечно-сосудистые заболевания с неблагоприятным, коморбидным невропатической акцентуации, расстройства личности течением. Выводы: при благоприятном течении сердечно-сосудистого заболевания в условиях конгруэнтной соматоперцептивной акцентуации — расстройство личности способствует улучшению прогноза заболевания (профилактика осложнений, высокая приверженность терапии).
