Современные лекарственные средства для фармакотерапии ожирения и метаболического синдрома (Обзор)

Аннотация

Целью данного литературного обзора явилось изучение возможностей современных групп препаратов для лечения ожирения и метаболического синдрома по повышению эффективности и безопасности фармакотерапии и снижению факторов риска развития диабета, кардиоваскулярных заболеваний, заболеваний опорно-двигательного аппарата и рака. Препараты для лечения ожирения, имеющие высокий риск развития побочных эффектов со стороны ЦНС и сердечно-сосудистой системы, должны использоваться исключительно для кратковременных курсов лечения. К таким средствам относятся антагонисты каннабиноидных рецепторов (римонабант), ингибиторы обратного нейронального захвата норадреналина и серотонина (сибутрамин), ингибиторы обратного нейронального захвата норадреналина, серотонина и дофамина (тезофизин), агонист 5HT2с рецепторов (лоркасерин) и другие лекарственные средства, влияющие на центральные и периферические нейромедиаторные процессы. Таким образом, для лечения ожирения предпочтительно применять более безопасные препараты метаболического типа действия, реализующие свой фармакологический эффект через влияние на абсорбцию жира из ЖКТ (орлистат), активирующие энергетические метаболические процессы (адипокины), ингибиторы метионин аминопептидазы 2 (MetAP2) (белораниб) и др. Комбинирование препаратов с разными механизмами действия увеличивает эффективность и безопасность терапии ожирения (Contrave®, Empatic™, Qsymia).
